Ervin Ray Polk

Ervin Ray Polk, Jr., 52, passed away on Sunday, July 5, 2015. He was born on Friday, April 12, 1963. Ervin was also preceded in death by his mother, Margaret Julia Matz and step-father, Mike Matz. Left to cherish his memory is his loving wife Linda Polk; two sons, Justin Ray Polk, Cody Ray Polk; brother, Mark Jonathan Polk; grandson, Mason Aubrey Polk; along with numerous other loving family and treasured friends. A Memorial Service for Ervin will be held on Saturday, July 18, 2015 at 2:00 pm at Neal Funeral Home.
